Who may benefit from Case Management Services?

  • Do you struggle with a Mental Illness, such as depression or anxiety?
  • Do you have Mainecare?
  • Do you have a difficult time accessing the community? Does your mental health compromise your level of community interaction? Do you find accessing transportation to be challenging?
  • Do you find yourself struggling with making and keeping medical appointments or following suggestions of medical providers?
  • Are you wondering how to access community resources?
  • Have you experienced homelessness or difficulty maintaining affordable housing?

If any of these questions resonate with you or with someone you know then you may benefit from Case Management services.

At Catholic Charities we offer three different types of case management services our intake clinicians will meet with you where is most comfortable for you.

What you may expect during an intake appointment:

Once you call us for an appointment or we receive a referral on your behalf you will be contacted by our intake team to schedule an intake appointment. At your intake appointment you can expect to be asked questions about your background, mental illness and current struggles. Due to the amount of information that must be gathered during this appointment, you can expect this meeting to last 1-2 hours. After intake is completed you will be assigned a case manager within 7 business days.

What you can expect from working with a Case Manager

Once you are assigned a case manager you will work together to create a personalized treatment plan that outlines your goals for case management services. You can expect that your case manager will support you with:

  • Coordinating transportation
  • Referrals to community resources
  • Applying for and maintaining federal and state benefits
  • Housing assistance
  • Referrals to medical and mental health providers
