To be eligible for formal enrollment into the PATH program, and individual must be homeless or at imminent risk of homelessness; have (or presumed to have) a mental illness or co-occurring disorder and not be currently supported by mainstream mental health services/providers. Once formally enrolled into the program, PATH’s goal is to expedite the individual’s transition to local community support providers. The program is also aimed at addressing the problem of homelessness among our veteran population.
Once outreach staff have identified a candidate is eligible for formal PATH services and there is mutual consent for other services to begin, the individual will be officially enrolled into the PATH program. All enrollees will be documented in order to measure program success and impact among the homeless community
