Assertive Community Treatment (ACT)

*Available only in Cumberland County

Support and Recovery Services in Portland Maine offers Assertive Community Treatment for individuals who require a higher level, team-based approach to mental health care. If you or your patient struggle with pervasive mental illness, frequent psychiatric hospitalization, chronic homelessness, criminal justice involvement or difficulty with medication adherence, our ACT Team may be able to provide continuity of service to meet your needs.

Services are delivered by the following multidisciplinary team:

  • Psychiatrist or Psychiatric Nurse Practitioner
  • Registered Nurses
  • Licensed Clinical Social Worker (Team Lead)
  • Substance Abuse Therapist
  • Vocational Specialist
  • Certified Intentional Peer Support Specialist
  • Comprehensive Community Support Worker (case manager)

ACT Team clients can expect to have their medication management needs overseen by our Psychiatric Nurse Practitioner and meet with various members of the team at least 3 times per week.
