Catholic Charities Maine Adult Mental HealthBehavioral Health Home (BHH)

Although “home” is included in the name, it is not a residential service. While similar to traditional Case Management, we utilize a multidisciplinary team. This team recognizes the importance of balancing both your physical and mental health. We work closely with your Primary Care Provider to ensure continuity of medical care in additional to addressing your mental health symptoms.

Services are delivered by the following multidisciplinary team:

  • Psychiatric and Medical Consultant
  • Registered Nurse Care Manager
  • Licensed Clinical Social Worker (Team Lead)
  • Certified Intentional Peer Support Specialist
  • Care Coordinators (case manager)

As a member of the BHH team we value your input. We encourage all members to participate in the development of the BHH programming and attend all groups offered by the BHH team.
