Children's Case Management
For at-risk children—and their families—life is difficult enough without the added struggle of coping with mental health issues, developmental disorders, and chronic medical conditions.
Licensed by the Department of Health and Human Services, the Children’s Case Management program serves eligible youth from birth to age 20. From assessment through advocacy and evaluation, we seek to:
- Encourage and build independence,
- Enhance functioning in home, school, and community
- Empower children to achieve their optimal potential
Children’s Behavioral Health Home Services are provided to children birth through age 20 with diagnosed developmental disabilities and mental health disorders. BHH services are team-based and include a nurse care manager, family support worker, youth support specialist, clinical team leader and consulting child psychiatrist in addition to the Health Home Coordinator.
In addition to our Case Management services, we offer Functional Family Therapy to help at-risk Maine children get the support and care they need to become strong, successful adults.
