How do I access services?

Walk-ins are welcome at 420 Cumberland Avenue in Portland, Monday through Friday 8:30 a.m. - 4 p.m. Ask to speak with Community Response.
Our Community Response Coordinator will assist you with completing a referral form, applying for insurance, if needed, and will schedule an appointment for your comprehensive assessment.
Referral information can also be taken by phone by calling (207) 871-7431 or requesting your current provider fax your referral form to (207) 871-7457.
